Application for the issuing of a duplicate qualification statement

• An original qualification will not be re-issued to a student in cases where the original document has been lost, damaged or destroyed.  Only a duplicate qualification statement will be issued.
• In the case of an original certificate being lost or destroyed, the applicant must submit an affidavit (a sworn statement that the certificate has been lost/stolen/destroyed, made at a South African Police Services Station)
• Complete the (application form).
• A fee of R211 per qualification must be paid into the following bank account number:
