Standard Operating Procedure:
1. Make sure you have the following documents ready to attach when completing the online form:
- Copy of positive identification: ID / Driver’s License / Passport
- Signed declaration by your service provider / employer
2. Complete the online application form available at: http://apps.ufs.ac.za/cardapplication/application.aspx
3. Submit the completed application form and attachments.
