Application for the issuing of a duplicate qualification statement

An original qualification will not be re-issued in cases where the original document has been lost, damaged or destroyed.  Only a duplicate qualification statement will be issued.

In the case of an original certificate being lost or destroyed, the applicant must submit an affidavit (a sworn statement that the certificate has been lost/stolen/destroyed, made at a South African Police Services Station).

Complete the application form.

A fee of R211 per qualification must be paid into the following bank account:
